Description
* Bachelor's degree in Speech-Language Pathology with an active CRFa license;
* Preferred postgraduate degree in Palliative Care and/or Gerontology;
* Experience working with specific populations, such as children, elderly individuals, or patients with special needs;
* Availability to work on-call for 150 hours/week as a daily shift from Monday to Friday, 1:00 PM to 7:15 PM.
* **MAIN DUTIES / RESPONSIBILITIES:**
\* Conduct clinical and functional assessments at the patient's home.
\* Define individualized therapeutic plans.
\* Collaborate with interdisciplinary teams to develop alternative assistance and treatment strategies, contributing to patient comfort.
\* Implement preventive, rehabilitative, and supportive interventions.
\* Monitor functional progression and clinical risk (dysphagia, communication, cognition).
\* Train family members and caregivers.
\* Conduct health workshops according to the company's scheduled programming and defined topics, aiming to promote a culture of disease prevention and improved quality of life.
\* Accurately document the entire care process.
\* Apply standardized institutional protocols for assessment and follow-up.
\* Update assessment tools in accordance with best practices and regulatory requirements.
\* Ensure third-party teams' adherence to technical protocols.
\* Analyze procedures performed by third parties, verifying compliance, progression, and clinical relevance.
\* Identify non-conformities and suggest improvements.
\* Participate in the development and review of SAD quality indicators.
\* Analyze, validate, and extend requests for speech-language pathology services.
\* Interface with third-party providers to discuss cases and align therapeutic plans;
\* Evaluate criteria for continuity of care, discharge, suspension, or change in care modality.
\* Coordinate with the multiprofessional team to determine the optimal therapeutic strategy for elderly patients.
\* Comply with Crefono resolutions, ANS regulations, and other applicable legislation.
\* Follow SOPs, internal guidelines, and patient safety policies.
\* Conduct home visits according to pre-established scheduling, making appropriate system entries to monitor the patient.
\* Generate performance indicators by entering required data into systems to support information development.
\* Evaluate, together with management and the Service Center, the quality of third-party services delivered.
\* Issue non-conformity reports for third-party services whenever failures or inconsistencies in service delivery are identified.
\* Issue technical opinions whenever necessary.
\* Record all actions in electronic medical records, internal forms, and email.
\* Interface with families regarding session adjustments, pending items, or changes, ensuring follow-up on related matters.
\* Document adverse events related to speech-language pathology services and home-based care, following internal safety and quality protocols.
\* Adhere to institutional routines, statutes, regulations, and current legislation;
\* Immediately notify the HR department of any changes to personal information: name, address, education level, marital status, etc.
\* Maintain a clean and organized workplace.
\* Comply with MedSênior's Code of Conduct.
\* Perform other related and/or inherent duties as described in the respective Professional Profile, Previdenciário Profile, and/or as requested by the immediate supervisor.

Compensation: R$4\.000,00 \- R$5\.000,00 per month
