




Description: * Bachelor's degree in Social Work (mandatory). * Postgraduate degree desirable in Patient Experience, Quality Management, Home Care, or related fields. * Flexible working hours. * Driver's license category B; personal vehicle required. * Conduct socio-environmental visits and map the social profile of patients and their families. * Propose actions to adapt the environment for home-based hospitalization. * Monitor cases requiring social intervention and support vulnerability-related actions. * Lead implementation meetings and follow up with patients across various assistance programs, including injunctions and discharge. * Monitor customer complaints through the Customer Service Center (SAC) and mediate conflicts. * Participate in clinical meetings with health insurance providers, presenting social reports and information. * Develop and monitor activities of the Humanization Project. * Support Quality Management System activities, including processes, flowcharts, and internal documents. 2512210202551931656
