This professional will work collaboratively as part of a multidisciplinary team, contributing to comprehensive care for patients and their families, with emphasis on active listening, social guidance, and socioeducational activities that enhance humanized care. **Responsibilities:** * Implement socioeducational and integration activities involving patients, family members, and the community; * Support the strengthening of familial and community ties, encouraging social interaction; * Promote activities that foster well\-being, qualified listening, and welcoming support; * Collaborate with the multidisciplinary team on strategies for health prevention, health promotion, and citizenship; * Prepare reports and documentation of implemented activities; * Participate in meetings, training sessions, and institutional projects. **Requirements:** * Completed high school education; * Technical or higher education qualification preferred; **Advantages:** * Experience in hospital settings or mental health; * Knowledge of public policies related to social assistance and health; * Ability to facilitate group activities and perform social mediation; * Experience in social work, healthcare institutions, NGOs, or public policy initiatives; * Strong communication skills, empathy, and ability to interact effectively with diverse populations and situations.
