




Job Summary: Professional responsible for verifying, organizing, and standardizing patient clinical and administrative documentation, ensuring document integrity. Key Highlights: 1. Organization and standardization of physical and electronic medical records 2. Verification and correction of mandatory documents 3. Support to various departments for document integrity 1 \- Verify patient clinical and administrative documentation; 2 \- Assemble, organize, and standardize physical and/or electronic medical records; 3 \- Verify the presence of mandatory documents (referral forms, prescriptions, progress notes, reports, test results, consent forms, and authorizations); 4 \- Identify pending items, inconsistencies, and missing information; 5 \- Request corrections and supplementary information from responsible departments; 6 \- Organize, archive, and digitize documents in accordance with institutional standards; 7 \- Ensure correct identification of patients and involved healthcare professionals; 7 \- Support billing, patient services, pharmacy, and clinical staff to ensure document integrity.
