Join our purpose! **Responsibilities and Duties** * Technically evaluate medical prescriptions according to established protocols; * Perform medication reconciliation; * Conduct pharmacotherapeutic monitoring of critically ill patients; * Investigate reports of adverse drug reactions; * Evaluate requests for non-standardized medications; * Evaluate medications brought by patients; * Periodically review—and whenever necessary—medication dilution tables, medications administered via enteral tube, and the high-alert medication manual; * Provide medication-related information upon request; * Technically evaluate parenteral nutrition prescriptions; * Physically inspect parenteral nutrition bags; * Audit compliance with good storage and distribution practices across all satellite pharmacies, monitoring storage conditions (temperature, humidity, hygiene, labeling, and location); * Evaluate/review requests for modifications to surgical kit compositions; * Monitor compliance with and effectiveness of expiration date control routines, and analyze daily counts of controlled substances; * Organize and participate in inventory activities; * Daily review of the surgical map against critical product stock levels to prevent stockouts. * Technical responsibility * Pharmacy team management and supervision * Development of standard operating procedures (SOPs) and protocols * Prescription monitoring and evaluation * Medication therapy optimization * Clinical pharmacy * Pharmaceutical care * Pharmacovigilance and technovigilance * Coordination of the Pharmacotherapy Committee and participation in unit-level committees * Requests for medication standardization and de-standardization * Supervision of medication repackaging * Requests for purchase of medications and related products * Management of emergency stock inventories * Participation in inventory activities **Requirements and Qualifications** * Completed degree in Pharmacy and active CRF (Regional Council of Pharmacy registration); * Specialization in hospital pharmacy is desirable; * Experience in hospital pharmacy; * **Availability to work administrative hours in Itaparica\-BA** Founded on February 21, 1937 by Professor José Silveira, the José Silveira Foundation (FJS) has expanded its mission through sustainable growth grounded in strategic planning and a focus on optimal outcomes.
