Job Summary: The professional will provide social assistance, identify needs, prepare reports, and mediate with the socio-assistance network, acting in discharge processes and family guidance. Key Highlights: 1. Social assistance and family guidance in a hospital setting. 2. Mediation with institutions and services of the socio-assistance network. 3. Participation in case discussions with the multidisciplinary team. **Job Description:** ---------------------- \- Provide social assistance to patients and their families; \- Identify and assess social needs related to treatment and continuity of care; \- Prepare social reports and opinions; \- Conduct mediation with institutions and services of the socio-assistance network; \- Participate in case discussions with medical and multidisciplinary teams; \- Act in hospital discharge processes (municipal, state, and interstate); \- Guide families on social rights, social security benefits, insurance, and procedures related to death; \- Request ambulances for transfers when necessary; \- Summon family members for alignment and follow-up regarding patient care. **Mandatory Requirements:** ------------------------------ * Bachelor's degree in Social Work; * Active registration with CRESS; * Experience in CRAS, CREAS, multidisciplinary residency, or hospital settings; * Knowledge of public policies and the socio-assistance network; * Strong communication skills, interpersonal relationship abilities, and emotional balance. **Desirable Requirements:** -------------------------- * Experience in hospital and/or oncology settings; * Knowledge of multidisciplinary practice. **Employment Type:** -------------------------- CLT **PwD (Persons with Disabilities):** -------- NOT APPLICABLE **Salary:** ------------ $5,912\.34 **Benefits:** --------------- * Health insurance; * Dental insurance; * Transportation allowance; * Food allowance; * Meal allowance; * Partnerships with educational institutions; * Wellhub (Gympass); * SESC partnership. **Work Location:** ---------------------- ASSOC DE COMBATE AO CANCER EM GOIAS **Working Hours:** ------------------------ 30h \| Mon-Fri \- 08:00 AM to 02:15 PM
