A proactive and humanized professional for technical welcoming, comprehensive diagnosis, and coordination with the mental health care network at a triage unit.

We seek a humanized and proactive professional to work at our triage unit in Rio das Pedras. The central objective is to conduct the initial technical welcoming, ensuring individuals receive comprehensive assessment prior to referral to the Shelter House.
Main Responsibilities
* Welcoming and Qualified Active Listening: Conduct individual and group sessions focused on emotional stabilization and initial diagnosis of those welcomed.
* Coordination with the Network (RAPS): Directly accompany welcomed individuals during CAPS consultations and procedures, serving as a bridge between the organization and public mental health services.
* Development of Multidisciplinary Clinical Records: Prepare and structure a detailed follow-up form, consolidating medical, psychosocial, and vulnerability histories.
* Diagnostic Flow Management: Ensure that, upon completion of the triage period, the welcomed individual arrives at the shelter house with a more complete diagnosis and a pre-established Individual Care Plan (PIA).
* Conflict Mediation: Address crisis situations within the triage unit, promoting an environment of respect and safety.
Requirements:
Bachelor's degree in Psychology with an active CRP registration.
Experience (or strong interest) in public policies, social assistance, and serving people experiencing homelessness.
Knowledge of SUS/SUAS functioning and the Psychosocial Care Network.
Technical writing skills for preparing reports and progress notes.
