




Job Summary: The professional will conduct speech-language pathology assessment, diagnosis, and intervention; develop individualized treatment plans; and collaborate with multidisciplinary teams. Collaborates with multidisciplinary healthcare teams. **Job Description:** ---------------------- Receive the patient and/or their legal guardian; collect clinical, social, and family history data (anamnesis); identify the primary complaint and care needs. Administer and conduct specific, comprehensive clinical and instrumental assessments to functionally analyze language (oral and written), speech, voice, hearing, and/or swallowing. Formulate the speech-language pathology diagnosis, issuing detailed technical reports and clinical records that justify the need for intervention. Develop and document the Individualized Treatment Plan (ITP), establishing short-, medium-, and long-term goals, intervention methodologies, and discharge criteria. Conduct rehabilitation and clinical intervention sessions (e.g., communication therapy, swallowing therapy), utilizing evidence-based techniques and resources. Select and train patients in the use of Augmentative and Alternative Communication (AAC) tools and other assistive technologies to enhance functionality and autonomy. Provide continuous health guidance and education to patients, family members, caregivers, and support teams regarding the diagnosed disorder and strategies to optimize treatment at home. Actively collaborate with multidisciplinary healthcare teams (e.g., physicians, nurses, physical therapists, occupational therapists), participate in clinical meetings, and ensure comprehensiveness and coordination of the Care Plan. Conduct regular follow-up and monitoring of patient progress, performing periodic re-assessments to measure advancement toward established goals. Adjust and revise the Individualized Treatment Plan (ITP) as needed, based on progress, regression, or emergence of new patient needs. Record, collect, and analyze productivity and quality data (indicators), ensuring delivery of departmental performance metrics.
