Provide social reception to patients and their families during psychiatric hospitalization; Conduct social interviews and sociofamilial assessments; Prepare social reports, opinions, and records in the electronic medical record; Collaborate with the multidisciplinary team in developing the individual therapeutic plan; Participate in clinical meetings and case discussions to formulate the Individual Therapeutic Plan (ITP); Guide family members on institutional routines, patients' rights, and continuity of treatment; Assist in admission, transfer, and discharge processes; Develop strategies to strengthen family bonds and social support; Monitor cases involving social vulnerability, family abandonment, or risk situations; Support requests related to judicial proceedings, documentation, and social referrals; Promote humanized care aiming at safety, ethics, and quality of assistance; Act in accordance with institutional regulations, Social Work legislation, and mental health policies; Contribute to quality indicators, hospital accreditation, and continuous improvement of processes; Participate in trainings, internal protocols, and institutional initiatives.
