Contribute to the preparation of periodic care line reports We are seeking a Nurse to plan, execute, and monitor longitudinal care for patients within the care line through structured nursing consultations, active case finding, indicator management, protocol development, and technical leadership of the team—ensuring clinical outcomes, program adherence, and operational efficiency. **Flexible schedule, Monday to Friday** **100% in-person work at Berrini (Temporary)** **We offer a comprehensive benefits package to take care of you:** Medical Assistance provided according to your eligibility Meal Voucher and Food Voucher for your daily meals TotalPass for your physical well-being Transportation Voucher to facilitate your commute Childcare Assistance, as per CLT regulations Dr. Consulta Card with exclusive health benefits **RESPONSIBILITIES:** * Conduct structured nursing consultations in accordance with the care line protocol * Develop individualized care plans with clinical and behavioral goals * Deliver targeted health education based on patient profile and risk level * Monitor clinical progression and therapeutic adherence * Request and follow up on examinations per institutional guidelines * Identify early signs of decompensation and refer to physician when necessary * Ensure patient safety throughout all stages of care * Identify eligible patients based on clinical criteria and abnormal test results * Perform active outreach for patients outside protocol or with inadequate outcomes * Lead technical assessment for inclusion into the care line * Provide longitudinal follow-up for active patients, focusing on clinical outcomes * Stratify risk and prioritize follow-up according to complexity * Address patients who miss appointments, experience decompensation, or have pending tests * Record information in a structured, indicator-oriented manner * Monitor clinical and operational indicators of the care line * Analyze clinical and operational performance indicators * Identify trends, bottlenecks, and improvement opportunities * Correlate clinical data with adherence and outcome results * Contribute to defining data-driven goals and action plans * Support patient acquisition campaigns and portfolio expansion strategies * Monitor program conversion and adherence rates * Demonstrate clinical outcome progression across the patient portfolio * Present acquisition, adherence, and productivity results * Support the preparation of periodic care line reports * Organize and present clinical and operational indicators during internal meetings * Lead evidence-based and institutionally grounded technical discussions * Propose continuous improvement plans * Participate in creating and structuring new care lines or subprograms * Develop strategies to expand coverage and adherence * Contribute to implementing innovative assistive technologies and workflows * Test and validate process improvements prior to scaling * Draft, review, and update Standard Operating Procedures (SOPs) * Standardize clinical and administrative workflows of the care line * Ensure compliance with technical and regulatory standards * Monitor compliance and propose adjustments when necessary * Guarantee adherence to care line clinical guidelines * Promote a culture of patient safety and quality care * Train staff on correct SOP application * Train nursing technicians and other team members on care line protocols * Conduct periodic training on clinical updates and care workflows * Develop internal educational materials * Monitor team technical performance and guide improvements * Collaborate integrally with physicians, nursing technicians, nutritionists, psychologists, and administrative staff * Participate in complex case discussions * Ensure alignment among clinical care, operations, and strategy * Maintain clear and structured communication among involved departments * Foster a collaborative environment focused on clinical outcomes **Requirements** * Active COREN registration * Experience in care line programs Minimum Education: Bachelor's Degree * Transportation Voucher * Food Voucher * Meal Voucher * TotalPass * Childcare Assistance * Medical Assistance
