Our job openings are also valid for persons with disabilities (PCD). \#MedSênior hashtag\#vagas hashtag\#hiring hashtag\#oportunidades hashtag\#tecnologia hashtag\#saúde hashtag\#contratando **Important: We do not charge any fees during our selection processes.** **Responsibilities and Duties** * Perform telemonitoring of Bem Envelhecer beneficiaries, ensuring quality guidance and assistance; * Provide program-related information and track health appointments together with the reference nurse; * Follow the monitoring schedule according to each beneficiary’s profile, adjusting deadlines when necessary; * Refer beneficiaries to MedSênior health centers and plan individualized care; * Alert the nurse in complex cases or those requiring greater attention; * Acquire patients through various sources (reports, post-hospital discharge, internal requests, etc.); * Monitor beneficiaries after hospital discharge and identify clinical, social, or familial warning signs, referring them to appropriate professionals; * Recommend and direct beneficiaries to health programs based on their frailty profile and Program Prime criteria; * Apply protocols, perform triage, and reassess health profiles whenever necessary; * Maintain active communication with beneficiaries and their families, document all interactions, and monitor adherence to therapeutic plans. **Requirements and Qualifications** * Currently enrolled in a Bachelor’s degree program in Nursing, from the 4th semester onward; * Proficiency in Microsoft Office Suite; * Residence in Greater Vitória; * Prior experience in telemonitoring is a plus; * Availability to work on-site. **Additional Information** Benefits: Meal allowance, Gympass, psychological support, birthday leave, and transportation allowance.
