




Evaluate the patient in the hospital, collecting data and hospital admission history, clinical status, and medical background; Engage with the healthcare team to clarify technical indications and possibilities for home-based care; Inform family members about the caregiver’s responsibilities and how they differ from those of a nursing technician; Conduct a social assessment to verify the presence of an eligible full-time caregiver, flagging any identified vulnerability; Introduce the company to the family, clarifying questions regarding home care services and assistance routines; Discuss the case to define the appropriate Home Care Program (PAD) to be provided; Clearly and objectively explain the PAD, including eligibility criteria; Prepare a clear, objective, and comprehensive report in the system, including all information related to technical discussions with the healthcare team and family engagement; Submit relevant documentation to each health insurance provider, along with Home Doutor’s liability forms and instructions for their completion. **Location:** Campinas Monday to Friday, 8:00 AM to 5:00 PM. Bachelor’s degree in Nursing and active COREN registration required.
