Job Summary: Nursing intern to provide patient care and follow-up, develop and update care plans, and administer clinical assessment scales. Key Highlights: 1. Administer and update clinical assessment scales and cognitive tests **RESPONSIBILITIES:** Conduct initial assessments for new patients, collaborating on the development of personalized care plans under the supervision of the registered nurse; Conduct follow-up consultations, assisting in maintaining and updating care plans; Respond to patient requests in person and by phone, providing guidance and support; Administer and update clinical assessment scales, such as the Clinical Frailty Scale, prediabetes assessments, Zarit Caregiver Burden Scale, cognitive tests, and the Geriatric Depression Scale (GDS); Monitor and respond to messages via WhatsApp, ensuring clear and organized communication; Schedule consultations and procedures, correctly assigning them to responsible professionals; Contact patients for follow-up on care delivery; Participate in meetings with the assigned nurse, contributing relevant information for continuous improvement of care; **WHAT WE OFFER:** Internship stipend of R$ 1.210,00; Life insurance; Transportation allowance according to need; Pharmacy benefit plan and SESC membership; **REQUIREMENTS:** Undergraduate nursing students who have already completed the Technical Semiology course; Responsible, committed, punctual, and proactive profile; **LOCATION:** R.
