Job Summary: A Social Work professional to work in a multidisciplinary context, focusing on welcoming, counseling, and supporting patients and their families. Key Highlights: 1. Participation in multidisciplinary rounds 2. Patient and family follow-up and support 3. Development of socioeducational activities Participate in multidisciplinary rounds and case discussions. Monitor patient transfers and readmissions, providing support to families throughout the hospitalization process. Conduct welcoming, counseling, and scheduling of visits for patients, families, and caregivers. Develop socioeducational activities and strengthen family and community bonds. Support and guide patients in regularizing their documentation (e.g., guardianship, power of attorney, vaccinations, proof of life, among others). Refer patients to support networks and community services as needed. Prepare social assessments, reports, opinions, and the Individual Care Plan (PIA), according to service requirements. Requirements: Bachelor's degree in Social Work, with active registration at CRESS. Strong written and verbal communication skills. Proficiency in preparing social assessments, reports, opinions, and other technical documents. Knowledge of planning, implementing, and evaluating socio-assistance actions and projects. Strong interpersonal skills, empathy, and ability to work in teams. Proactive, resilient, committed profile with strong mediation and communication abilities.
