**Job Objective** Support the clinical care department in caring for patients enrolled in CaptaMed’s home-based programs, working under direct supervision to contribute to process organization, quality of care, and patient safety. **Main Activities and Responsibilities** * Support clinical care department activities under supervision; * Conduct telephone contact with patients and family members for monitoring; * Assist in scheduling medical and nursing appointments; * Perform home visits and low-complexity procedures when accompanied; * Enter medical and multidisciplinary progress notes into the system; * Assist in transcribing medical prescriptions into the IW System; * Support the release and control of materials and medications; * Manage bed-stock inventory at the patient’s home; * Assemble clinical records; * Assist in extension processes (budgets, addendums, authorizations, and portal entries); * Record complications, incidents, and billing pending items in the system; * Support requests to health insurance providers, hospitals, and clinics; * Participate in departmental meetings and planning activities; * Contribute to Quality Management System initiatives, including notifications, non-conformities, and continuous improvement. **Requirements** * Currently pursuing a degree in **Nursing**; * Proficiency in **computer skills (Microsoft Office Suite)** preferred.
