Promotion of school and social inclusion OSID – Obras Sociais Irmã Dulce is one of the largest philanthropic institutions in Brazil, performing approximately 5.6 million outpatient procedures annually in Bahia for users of the Unified Health System (SUS), elderly individuals, persons with disabilities and craniofacial deformities, people experiencing homelessness, users of psychoactive substances, and children and adolescents at social risk. The organization offers a unique service profile in Brazil, distributed across 21 centers providing assistance to low-income populations in the areas of Health, Social Assistance, Scientific Research, Health Education, Education, and preservation and dissemination of the history of its founder, Saint Dulce of the Poor. **Responsibilities and Duties** 1\. Collaborate within an interprofessional team; 2\. Provide services to persons with physical, intellectual, hearing, visual, and multiple disabilities, respecting their diversity; 3\. Participate in interprofessional meetings for planning, monitoring, and evaluating service proposals and clinical case discussions, including preparation of Individualized Therapeutic Plans (ITPs); 4\. Provide welcoming services, support, and guidance to family members and caregivers of patients; 6\. Plan, organize, and facilitate group sessions for patients and their families; 5\. Diagnose and assess functionality; 6\. Coordinate with other components of the Care Network for Persons with Disabilities (primary care, hospital care, and emergency/urgent care); 7\. Coordinate with social protection, education, sports, culture, and other services; 8\. Conduct assessments for assistive technology use by persons with disabilities; 9\. Perform visual assessments and training on optical and non-optical resources, as well as orientation and mobility training for persons with visual disabilities; 10\. Carry out activities and follow-ups aimed at inclusive schooling from the perspective of inclusive education; 11\. Deliver pedagogical services that foster lifelong learning, considering aspects of social and professional inclusion; 12\. Develop evaluation instructions and tools; plan, organize, and implement continuing education strategies covering various topics related to persons with disabilities, targeting patients, family members, teachers, and professionals from diverse fields; 13\. Plan and conduct matrix support activities through coordination to exchange information and share responsibilities in managing ITPs across the network; 14\. Plan and carry out external activities promoting participation and social inclusion of persons with disabilities, guiding and supervising educational instructors. 15\. Guide and supervise Educational Instructors in Therapeutic Workshop activities. 16\.
