If you identify with our mission and seek professional growth and development, let’s work together! **Responsibilities and Duties** **Main Responsibilities:** * Accompany patients and their families/legal guardians, aiming to minimize the social impacts arising from hospitalization. * Coordinate with the family and intersectoral support network (CAPS, Programa Melhor em Casa, CRAS, CREAS, UBS, municipalities, secretariats, among others) to discuss cases and ensure safe hospital discharge. * Collaborate with the multidisciplinary team on the hospitalization process, ensuring patients’ and their families’/legal guardians’ rights and access to information within the health\-illness context. * Prepare social reports and opinions for the Judiciary and intersectoral network, safeguarding social rights. * Encourage patient and family/legal guardian participation in processes related to hospitalization, promoting ongoing support and understanding of health treatment. * Raise awareness among families/legal guardians regarding the need for support and follow-up during the post\-discharge period, promoting patient recovery and health prevention. * Actively participate in case discussions during huddles held in inpatient units, serving as a reference in support groups. * Support academic students and residents. * Conduct social assessments and social studies to inform planning for patients’ hospital discharge outcomes. **Requirements and Qualifications** **Requirements and Qualifications:** **What we are looking for:** * A professional aligned with our purpose of excellence in patient care. **What you need to have:** * Completed undergraduate degree in Social Work; * Specialization or residency in the field is desirable. **Additional Information:** * Professional experience in this area is an advantage; * Working hours: 12 p.m. to 6 p.m.; * Shift schedule: 5 days on, 2 days off; * Modality: On-site. **Additional Information** **Join our team!** We are a Marista institution that believes in the potential of our employees.
