Carry out social service activities, providing care to patients and their families in social, economic, and cultural aspects related to the health\-illness process, proposing measures that contribute to the implementation of medical treatment. **Responsibilities and Duties** * Provide social services to patients of the Institution, identifying and analyzing material, emotional, psychological, or other types of problems, applying basic social work processes to assist in patient recovery and promote reintegration into social, family, and work environments; * Diagnose and develop intervention plans regarding the social and emotional aspects of the healthcare team\-patient relationship through social interviews conducted at patients’ bedsides, aiming to provide psychosocial support and continue case follow\-up; * Maintain the emotional balance of the patient and/or family through support, modifying attitudes and seeking solutions; * Develop an annual plan outlining actions and initiatives to be implemented, designed, and/or proposed; * Facilitate communication between the healthcare team and patients’ families, enabling contact between both parties for clarification regarding diagnoses and treatments to be performed; * Assist patients throughout various stages of illness (hospitalization, convalescence, and community reintegration); * Guide patients’ families upon discharge, death, or other events through individual or group family interviews, providing psychosocial support; * Attend to patients dependent on oxygen therapy and home hospitalization by interviewing\-them and their family members, guiding them toward available resources offering such services and referring them accordingly, aiming to improve quality of life and prevent rehospitalization; * Attend to victims of abuse, conducting interviews with patients and their families, maintaining contact, and forwarding reports to the Juvenile and Children’s Court, Police Station (CT), Elderly Council, CRAS (Centro de Referência de Assistência Social), and Social Promotion; * Attend to women victims of sexual abuse and other forms of violence, providing psychosocial support to patients and their families; * Attend to indigent patients by contacting municipal shelters to secure vacancies and arranging transportation to these shelters following hospital discharge; * Request ambulance transportation for patients from their municipality of residence for discharge cases; * Accompany family members during patient transfer from Adult and Pediatric ICUs, preparing reports and respective internal referrals; * Evaluate and discuss cases with the multidisciplinary team to gain comprehensive understanding of the diagnosis and proposed treatment. **Requirements and Qualifications** * Completed undergraduate degree in Social Work; * Active CRESS registration; * Prior experience in the field is desirable. **Additional Information** Work schedule: Monday to Friday, from 10:00 AM to 4:00 PM **SPDM** \- Associação Paulista para o Desenvolvimento da Medicina is a non\-profit civil association of philanthropic nature, recognized as being of federal, state, and municipal public utility — respectively by Decrees No. 57\.925 dated 03/04/1966, No. 40\.103 dated 05/17/1962, and No. 8\.911 dated 07/30/1970. It is a low\-complexity emergency care unit owned by the Municipal Government of São Paulo, designed to handle spontaneous demand and equipped with 54 observation beds across specialties including General Surgery, Internal Medicine, Obstetrics and Gynecology, Orthopedics and Traumatology, and Pediatrics, managed by **SPDM** — Associação Paulista para o Desenvolvimento da Medicina — under a strong partnership with the Municipal Government, aligned with the guiding principles of Brazil’s Unified Health System (SUS), ensuring comprehensive, high\-quality hospital care
