Job Summary: A nursing professional responsible for coordinating care, developing therapeutic plans, delivering clinical care, and managing health care programs. Key Highlights: 1. Participate in multidisciplinary meetings and training sessions **Description and Responsibilities:** **Schedule:** 8:00 AM to 5:58 PM, with a 1-hour and 10-minute break, Monday through Friday. **Level:** Professional **Employment Type:** Permanent – CLT * Assess patients eligible for health care programs according to pre-established protocols. * Conduct in-person admissions by applying the enrollment agreement and performing patient anamnesis, as stipulated by the Specialized Care Network programs. * Perform patient triage, when necessary, for the Specialized Care Network programs. * Coordinate care for patients participating in the programs. * Schedule appointments and procedures according to the programs’ standard care routines. * Develop nursing therapeutic plans. * Engage the multidisciplinary team based on patient assessment during nursing consultations. * Evaluate patients for program transfer, according to clinical status (discharge or clinical deterioration). * Conduct and mediate family meetings. * Perform nursing on-call duties for monitoring patients in the Specialized Care Network, per departmental scheduling. * Deliver clinical care to patients at CAS or home, performing nursing procedures to support the medical team and meet service demands. * Encourage greater participation of patients and their families in the proposed treatment, promoting health education. * Guide and train caregivers and families on self-care measures for the patient. * Conduct in-hospital consultations for active case finding and safe discharge. * Develop the Home Care Plan for discharge. * Record all activities in the electronic medical record according to established protocols. * Provide patient care, ensuring comfort and well-being, while adhering to best practices, safety norms, and procedures. * Pursue frequent updates within the field of practice. * Promote actions that foster and sustain a positive organizational climate. * Develop, implement, operate, and execute projects defined jointly with area management, aligned with the cooperative’s and internal/external customers’ needs, ensuring adherence to timelines and goals. * Perform equipment washing, cleaning, and disinfection according to institutional schedules. * Regularly inspect medications and solutions used in clinics and outpatient settings, and properly discard expired items. * Participate in multidisciplinary team meetings and case discussions. * Participate in events promoted by the Cooperative as a speaker or contributor to informational programs (TV, radio, interviews). * Conduct operational meetings with third-party providers. * Deliver necessary training and guidance to providers. * Participate in the Sector’s Continuing Education Program. * Supervise less-experienced professionals, supporting task execution, providing guidance, and offering suggestions to improve routines and processes. * Perform administrative support activities related to the specific area of responsibility, following established guidelines and goals to ensure timely and accurate execution. * Submit mandatory notifications. * Report to Coordination any need for pest and vector control, equipment maintenance, or urgent/emergency situations. * Enter medical prescriptions into the system for provider execution. * Evaluate operational indicators for programs under responsibility, conducting critical analysis jointly with Coordination. * Transfer work processes and routines to new employees. * Update and manage processes according to the programs’ Standard Operating Procedures (SOPs) to ensure medical record quality. * Create and update SOPs and sector documents. * Evaluate performance of third-party contracts. * Propose improvements to eligibility criteria for health care programs. * Monitor and analyze high-cost patient alert calls from Unimed Cascavel’s portfolio and develop action plans. * Perform other related duties consistent with the nature of the role, as directed by superiors. **Requirements:** * Completed undergraduate degree in Nursing. * Active COREN registration. * Knowledge of Nursing Procedures. * Skill in Humanized Care. * Basic knowledge of Palliative Care.
