Register and submit your resume! **Responsibilities and Duties** ----------------------------------- * Provide social services to patients and their families, performing duties inherent to the profession; * Participate in the interdisciplinary team, collaborating in the development of the patient’s therapeutic plan; * Conduct social assessments; * Address and monitor cases involving suspected or confirmed social vulnerability; * Provide guidance, referrals, and follow-up for users regarding social security benefits, social assistance programs, documentation, and other matters; * Attend interdisciplinary meetings and visits at the unit; * Provide bereavement support and accompany family members by offering assistance, guidance, and emotional support; * Guide and monitor cases related to patient transfer and hospital discharge; * Mediate conflicts and respond to spontaneous requests. **Requirements and Qualifications** **Mandatory:** * Bachelor’s degree in Social Work; * Registration with the professional regulatory council; * Proficiency in computer applications and Excel; * Knowledge of health legislation and social rights legislation. **Desirable:** Behavioral Profile * Effective and empathetic communication skills; * Ability to work effectively within an interdisciplinary team. * Specialization in health; * Professional experience in a hospital setting.
