Job Summary: The Social Work professional will ensure humanized assistance, identifying and intervening in situations affecting the therapeutic plan, and integrating inter-sectoral teams. Social follow-up and planned interventions Ensure humanized assistance to users and their families, identifying situations that may interfere with the implementation of established therapeutic plans, applying principles and techniques of Social Work while considering the legal and social protection fields, and integrating and contributing with inter- and extra-sectoral teams to resolve problems. **Schedule:** 1:00 PM to 7:00 PM (Monday to Friday) **Salary:** R$3237.74 **Benefits:** * Transportation allowance; * Meal voucher or food allowance; * Health insurance with no monthly fee (UNIMED); * Dental insurance with no monthly fee (METLIFE); * Educational assistance (Undergraduate and Postgraduate); * Daycare allowance – R$200.00 per child, subject to eligibility criteria and for children up to 48 months of age (limited to 3 children), intended for: o Mothers or legal guardians of children up to 48 months of age, or of children with special needs of any age; o Fathers with definitive custody of children up to 48 months of age, or of children with special needs of any age. * Quality-of-life program; * Funeral plan; * Partnerships with restaurants, Pacheco, psychologists, pharmacies, auto repair shops, and gyms; * Christmas gift card. **Responsibilities and Duties** While providing assistance: * Prioritize Social Work services for health users and companions facing social vulnerability; * Follow up on hospitalized users through inter-consultation requests, assessing social demands that may interfere with treatment and the discharge process; * Conduct observation, reception, and social follow-up; * Carry out diagnostic investigations for planned social interventions, applying Social Work methods and techniques; * Perform socioeconomic assessments, referring users/families to social inclusion projects and/or programs aligned with their needs; * Inform and guide users/families about institutional rules and routines; * Guide and refer patients to obtain public network social resources and benefits, as well as support entities, to ensure continuity of treatment; * Identify and monitor users undergoing prolonged hospitalization, intervening with the healthcare team to expedite treatment and subsequent discharge; * Arrange transportation through competent Municipal and State Health agencies—or directly through the Institution—to meet user needs and optimize the hospital discharge process; * Support responsible discharge by contacting and referring users to their primary care reference network, issuing and forwarding counter-referral reports (as requested by governmental, non-governmental, and involved institutional departments – EMAD/UBS/PSF/CREAB); * Request supplies, special diets, equipment, or any socio-assistance materials from the Brazilian service network structure, regardless of administrative level (municipal, state, or federal), aiming to sustain necessary follow-up/treatment; * Attend hearings when summoned by the Public Prosecutor’s Office; * Guide and support families and/or guardians regarding issues arising from deaths, working collaboratively with the multidisciplinary team; * Implement actions enabling burial of users when family cannot be located; * Conduct shift handovers/care transfers; * Document all cases followed by Social Work in medical records. * Participate in and lead training sessions, administrative meetings, multidisciplinary rounds, assistance groups, and other institutional projects, as required; * Supervise and mentor Social Work students, providing learning opportunities consistent with curriculum requirements and institutional guidelines. **Requirements and Qualifications** Bachelor’s degree in Social Work; postgraduate studies, specialization, or master’s degree preferred; Knowledge or coursework in management or project development is desirable; Active registration with CRESS-MG. **Additional Information** Experience in pediatric hospital settings is desirable.
