Florence Clinic, the first transition unit in the North and Northeast regions, is seeking a Temporary Social Worker. **Responsibilities and duties** * Provide support and welcome to patients and families during the adaptation phase at the clinic for adherence to the proposed treatment; * Offer guidance to families regarding institutional rules and routines; * Participate in family meetings to discuss the therapeutic plan, hospital discharge planning, and alignment of profiles for palliative care; * Provide support to families of patients who pass away; * Coordinate and supervise events requested by patients and families in the communal space; * Assist communication professionals with necessary information regarding patients, families, and humanization initiatives carried out at the clinic (filming, interviews, testimonials, etc.); * Actively participate in all processes related to patient intake. **Requirements and qualifications** * Completed undergraduate degree in Social Work; * Active CRESS registration; * Solid experience in patient/family/team follow-up; * Desirable experience in dehospitalization processes; * Strong empathetic communication skills with patients, families, and team members; * Advantageous to have training in Difficult Communication.
