




Provide social follow-up for patients and their caregivers, conducting assessments, guidance, and documentation in accordance with institutional protocols and current legislation, with a focus on quality of care. **Responsibilities and Duties** * Conduct interviews and social assessments of patients and their caregivers; * Collect and analyze information to define interventions and referrals; * Prepare technical reports, opinions, and documents relevant to the role; * Participate in case discussions with the multidisciplinary team and supervision sessions; * Provide guidance and feedback to caregivers, where applicable; * Maintain up-to-date records in the electronic medical record; **Requirements and Qualifications** * Bachelor’s degree in Social Work, with active professional registration; * Specialization in health is desirable; * Intermediate proficiency in computer tools (Microsoft Office suite). **Additional Information** * Administrative working hours from Monday to Friday. * Salary + market-competitive benefits
