Responsibilities and Duties: * Receive evaluation requests from health insurers and forward them to the evaluating nurse to ensure evaluations are conducted as soon as possible; * Monitor response time for evaluations performed in hospitals or at home; * Conduct case discussions to define the appropriate PAD (Patient Admission Document) for each case; * Ensure submission of cost estimates to health insurers (OPS) within contractual guidelines and within 24 hours; * Follow up on submitted cost estimates; * Analyze rejected cost estimates; * Ensure all information is accurately recorded in the IW system; * Coordinate with hospital discharge departments; * Maintain strong relationships with health insurers (OPS); * Submit the pre-implementation plan and follow up accordingly; * Provide all necessary resources available to ensure safe patient implementation; * Transcribe PM (Medical Prescription) and PE (Nursing Prescription) for hospital discharge; * Guarantee patient implementation at home within company-established deadlines; * Monitor implemented patients for 48 hours; * Hand over implemented patients to the case manager during shift change;" Requirements and Qualifications: * Completed Bachelor's degree in Nursing, with active COREN registration; * Postgraduate qualification in auditing or management is desirable; * Minimum 5 years of experience in home care, transition clinics, or ILPIs (Institutions for Long-Term Care of the Elderly); * Operational knowledge of the IW system is desirable.
