We seek students passionate about health care and interested in supporting elderly individuals through monitoring and assistance, contributing to rehabilitation, treatment, and disease prevention—all while prioritizing quality of life and well-being. **Your typical day:** * Conduct initial assessments for new patients, collaborating under the supervision of the responsible nurse to develop personalized care plans; * Conduct follow-up consultations, assisting in maintaining and updating care plans; * Address patient requests in person and by phone, providing guidance and support; * Apply and update clinical assessment tools, such as the Clinical Frailty Scale, prediabetes assessments, Zarit Scale, cognitive tests, and the Geriatric Depression Scale (GDS); * Monitor and respond to WhatsApp messages, ensuring clear and organized communication; * Schedule consultations and procedures, directing them appropriately to responsible professionals; * Conduct follow-up calls with patients to monitor care progress; * Participate in meetings with the assigned nurse, contributing relevant information to continuously improve care delivery; **What we seek:** * Undergraduate nursing students who have already completed the Technical Semiology course; * Responsible, committed, punctual, and proactive profile; **What we offer:** * Internship stipend aligned with market standards; * Transportation allowance. **Work schedule:** Monday to Friday, from 1:00 PM to 7:00 PM, plus one Saturday per month according to the rotating schedule.
