In the meantime, keep your information updated and complete all online assessment stages so that we may learn more about you. **Responsibilities and duties** About the position: * Identify patient and/or family needs through qualified active listening, in order to propose solutions and make necessary referrals. * Clarify hospital routines to patients and families via direct assistance, aiming to highlight both the rights and responsibilities of the hospital and those of the patient and companion. * Intervene in cases where the companion lacks financial resources to cover meals/accommodation, through internal and external mediation, aiming to ensure access to such services for companions. * Support vulnerable patients (victims of violence/abuse, elderly, persons with disabilities) upon request by the attending physician, aiming to promote their social empowerment and access to the Rights Protection Network. * Counsel patients requesting discharge against medical advice, voluntary discharge, or refusing treatment, through individualized counseling, clarifying risks associated with discontinuing care and arranging, when requested, transfer to another hospital per patient’s preference. * Support families of deceased patients by providing information, aiming to prevent bureaucratic or social complications. * Collaborate with the multidisciplinary team to anticipate social work needs, such as assistance with discharge planning, exceptions to ICU visiting hours, supervised visits for minors, and other opportunities. * Assist with or complete SUS reports and requests for enteral nutrition, oxygen therapy, and high-cost medications. * Provide emotional support to families at the time of death, addressing the possibility of organ and tissue donation. **Requirements and qualifications** If you believe in compassionate, holistic care and wish to transform global healthcare, our care services area offers an environment rooted in care and support.
