Coordinate with public and private healthcare networks (Home Care, provision of diapers, diets, equipment, oxygen, transportation, medications, etc.) to ensure patients' rights. Work according to the Expanded Visit concept, promoting family and support network integration during hospitalization. Propose and implement actions that encourage social participation of patients and families. Perform hospital referral management in coordination with the healthcare team, focusing on public or private referrals (psychiatric cases, chemical dependency, and victims of violence). Organize patient transfers to other public or private healthcare facilities, according to medical indication or family request. Provide public service (patients and families) for spontaneous requests or referrals from the healthcare team. **Desirable Requirements** Degree in Social Service. Minimum of 6 months of experience in Social Assistance and/or Public and Private Health services. Skills in customer service and conflict resolution.
