Description

* Ensure compliance with legislation and the supply policy of the SC Network applicable to hospital pharmacy activities; * Support technical leadership in implementing sectoral operational routines; * Enforce current legislation regarding storage, preservation, inventory control of medications, health products, sanitizers, supplies, and raw materials, as well as regulations related to their distribution and use; * Ensure implementation of the medication and hospital material dispensing and transportation system with traceability; * Ensure implementation of safe dispensing processes, focusing on prevention and/or detection of errors in the medication use process, guaranteeing that the correct medications reach the patient safely, at the right time, and in the appropriate dose; * Perform validation of the drug fractionation process; * Ensure implementation of standard operating procedures and completion of designated forms; * Interact interdisciplinarily with teams when requested; * Conduct pharmacovigilance and technovigilance activities, reporting suspected adverse reactions and technical complaints through the internal notification channel; * Support educational and training activities for the pharmacy team; * Guide the team and ensure proper management of waste generated from technical activities conducted in the hospital pharmacy, complying with sanitary and occupational health regulations; * Perform and maintain records of pharmaceutical activities, observing current legislation; * Support inventory execution; * Assist in developing work schedules and monitor their implementation; * Ensure accurate dispensing and control of medications included in Ordinance No. 344/1998 issued by the Ministry of Health and its updates, as well as their documentation; * Provide technical information on pharmaceutical operations, including preparation and dilution of germicides, reconstitution of medications, and preparation of intravenous mixtures/parenteral nutrition during drug fractionation, where applicable; * Monitor decisions related to drug therapy, such as application of clinical protocols, medication use protocols, and prescriptions; * Conduct clinical activities focused on patient care and rational use of medications, including participation in clinical rounds when indicated, pharmacotherapeutic monitoring of prescriptions, medication reconciliation, pharmacotherapy optimization, and provision of discharge counseling.
