Follow-up and rehabilitation of patients across various specialties. **Description:** Speech-Language Pathologist\- Speech-Language Pathology\- Concórdia Unit **Salary:** 4\.275,23\+ Benefits **Schedule:** 7:00 AM to 1:00 PM, Monday to Friday **Required:** Completed undergraduate degree in Speech-Language Pathology / Experience in adult/geriatric hospital speech-language pathology **Desirable (Plus for candidates; not mandatory for hiring):** experience in hospital oncology cases and dysphagia / experience in multidisciplinary clinical settings (oncology, cardiology, internal medicine, ICU) Assess and monitor patients who have received interconsultations via the Gesthos and outpatient oncology systems; participate in bedside rounds with the team when necessary; monitor the care plan and implement follow-up actions to achieve patient improvement goals; assess swallowing alterations; verify whether extubated patients are eligible to begin speech-language pathology intervention targeting swallowing and communication disorders, including tracheostomy decannulation. Assist in managing departmental documentation (indicators, SOPs, and other relevant documents). Provide services at Santa Lúcia Unit upon coordination request. Assess patients who have received interconsultations via the Gesthos system; review medical records to evaluate each patient, analyzing current and past medical history; perform direct and indirect swallowing assessments to identify dysphagic patients or those at risk for dysphagia, enabling timely initiation of speech-language pathology interventions to prevent aspiration risks; for patients already cleared for oral intake but still at risk for dysphagia, provide ongoing monitoring to suspend or adjust food consistency jointly with physicians and clinical nutritionists; assess extubated patients’ eligibility for initiating speech-language pathology rehabilitation, in collaboration with physicians, aiming to optimize oral feeding and communication (voice, speech, and language); monitor patients’ daily progress and clinical evolution via the Gesthos system, bedside visits, multidisciplinary team meetings, and care plans to ensure updated information, formulation, and implementation of therapeutic plans; following direct and indirect assessment, determine the appropriate treatment modality; develop targeted therapies for swallowing disorders and tracheostomy decannulation to support rehabilitation; discuss clinical cases with the care team; if thickener use is deemed necessary, communicate this to the attending physician for prescription, or prescribe it directly when authorized; document all patient encounters in the Gesthos system, print reports, and file them in physical medical records; schedule all patients for appointments in the GestHos system; after documenting progress, verify whether the appointment has been completed; if unable to see the patient on the scheduled day, reschedule the appointment; deliver care per the therapeutic plan, continuing treatment in the ICU and inpatient units to achieve full swallowing rehabilitation; initiate oral diet according to patient assessment and clinical progression; instruct caregivers on feeding-related precautions—such as patient positioning during feeding, level of alertness, consistency of prescribed diet, and thickener usage—to ensure safety and continuity of care throughout the feeding process; complete the care plan according to institutional scope requirements; fill out the department’s Daily Patient Monitoring Worksheet; attend departmental meetings; participate in institutional training programs; join institutional committees and initiatives upon coordination request; read emails daily; monitor supply inventory—including dyes and office supplies—and request replenishment when shortages occur, promptly notifying coordination via email; track incidents—including incident reporting, responses, and action plans—in the GestHosWeb Incident Module; record and monitor departmental risks per Risk Management protocols, as requested by coordination; generate service orders for responsible departments in case of access issues, verifying the required workflow; if coordination intervention is needed, notify them personally or via email; prepare/complete monthly departmental indicators, risk management documentation, annual training plans, printed “Management at a Glance” boards, or any other workflows requested by coordination or the institution; in outpatient settings, conduct direct and indirect speech-language pathology assessments for oncology patients, analyzing current and past medical history; document progress in the Gesthos system for all treated patients and print reports for submission to the Oncology Secretariat or SUS outpatient clinic; continuously monitor patients for dysphagia risks; attend Oncology Department meetings (inpatient and outpatient); monitor patients beginning cisplatin-based chemotherapy regimens in Head and Neck clinics to prevent ototoxicity; consult the Assistant Management team in the absence of coordination and execute established departmental workflows according to scheduled deadlines; travel to other units affiliated with or potentially joining the São Francisco Hospital Foundation, as directed by coordination or management. 2511060202271768940
