Monitor patients' pharmacotherapy **Description:** We are seeking an On-Duty Pharmacist to work in our hospital, providing pharmaceutical care support to the healthcare team and ensuring accurate medication dispensing to patients. The professional will be responsible for ensuring the smooth operation of the hospital pharmacy during shifts, promoting medication safety and appropriate use. **Responsibilities:** Dispense medications and hospital supplies according to medical prescriptions; Monitor patients' pharmacotherapy, providing support to physicians and nursing staff; Advise on correct medication use, dosage, and adverse effects; Ensure proper storage and control of medications in compliance with current regulations; Participate in medication inventory management and expiry date control; Monitor and ensure medication use safety, preventing medication errors; Perform other pharmacy-related duties during shifts. **Requirements:** Bachelor’s degree in Pharmacy; Registration with the Regional Council of Pharmacy (CRF); Prior experience as a hospital pharmacist (preferred); Availability to work rotating shifts and on-call duties, including weekends and holidays; Ability to work under pressure, attention to detail, and strong interpersonal skills.
