Job Summary: This professional will be responsible for providing pharmaceutical care, auditing units, monitoring medications, and participating in quality and patient safety management. Must be available to serve all pharmacies of the Institution as determined by management; • Provide assistance to both units; • Review the pharmacists’ daily report notebook, report any incidents to the Pharmacy Coordination, and document necessary information in the notebook to ensure continuity of care; • Read and respond to emails daily; • Conduct audits in clinical units, warehouses, pharmacies, and the central dilution unit; • Verify whether any Non-Standard Requests are pending authorization from Pharmacy Coordination or the Executive Directorate; • Verify whether any Non-Standard Medication treatments are nearing completion and discuss with the medical team the need for continuation; • Monitor patients receiving Non-Standard Medications, Restricted-Use Standard Medications, medications brought from home, and medications dispensed by the State Health Department; • Substitute for pharmacy supervisors in their duties whenever necessary; • Deliver announcements to clinical departments; • Develop clinical pharmacy and patient safety indicators; • Participate in the development, evaluation, and implementation of quality processes (e.g., SOPs, Process Maps, Risk Management) through meetings and training sessions conducted by the department to ensure service quality; • Assist in managing MAT/MED expiry dates across satellite pharmacies and medication carts, collaborating with other units or hospitals as needed; • Conduct training sessions for staff; • Perform daily prescription reviews and implement necessary interventions; • Document daily progress notes in the electronic medical records of patients served or monitored by the clinical pharmacy; • Email VISA-related notices to pharmacists at both units; print and file them in the corresponding folder.
