




Job Summary: Nutrition intern to perform screening, classification, and nutritional assessment; define discharge plans; and participate in multidisciplinary discussions. Key Highlights: 1. Definition of hospital discharge plan and patient counseling * Conduct nutritional screening of patients receiving oral therapy according to age group. * Classify these patients into levels of nutritional care to determine follow-up requirements. * Make appropriate menu adjustments and assess the need for supplementation for patients classified as yellow or red level. * Perform daily nutritional assessments of patients receiving enteral therapy to verify diet acceptance and tolerance. * Participate in clinical case discussions with the multidisciplinary team. * Define the hospital discharge plan from patient admission onward, counseling the patient and their family regarding post-discharge care.
