CLT Belo Horizonte / MG ##### **Description** Technically coordinate the patient portfolio; develop and review care plans; monitor patients' clinical and functional progress; use standardized clinical assessment scales (Barthel, Braden, Glasgow, NEAD, and others); identify transitions in care profiles (rehabilitation, maintenance, discharge, or home hospitalization); assess clinical needs for equipment, supplies, and therapy; proactively address avoidable clinical risks and complications; manage complications according to protocol, escalating cases outside standard parameters; support the multidisciplinary team in delivering care; monitor clinical dashboards of the unit; monitor home inventory data recorded by frontline staff, validating inconsistencies and escalating requirements to headquarters; produce technical opinions for PAD realignment when necessary; participate in technical meetings with Clinical Care and Clinical Governance teams; ensure care delivery is grounded in clear clinical criteria and evidence-based practices; escalate clinical exceptions to the Clinical Care Manager; conduct targeted patient visits for clinical auditing, care validation, and care alignment; deliver technical training and on-site supervision as needed, among other routine responsibilities. ##### **Requirements** * Completed Nursing degree / Active COREN registration / Postgraduate qualification or specialization in fields related to clinical care, health management, home-based care, emergency and critical care, or related areas. ##### **Benefits** \- Transportation allowance * Food allowance * Totalpass * Telemedicine * Birthday Day Off * Health insurance plan * Job opening date: 05/13/2026
