Job Summary: We are seeking a Home Care Nurse to lead comprehensive patient care in home-based healthcare settings, coordinate a multidisciplinary team, and ensure safe, humanized care. Serve as the key interface among family, care team, and health insurance providers We are a home healthcare company that delivers compassionate, technically excellent care—and we seek a Home Care Nurse to lead comprehensive care for our patients, working side-by-side with a welcoming, collaborative, and committed multidisciplinary team dedicated to delivering safe and humanized care within each family’s home. **Responsibilities and Duties** * Conduct initial patient assessment and develop an individualized Care Plan; * Coordinate and manage care delivery for beneficiaries under your responsibility; * Monitor the patient’s clinical progression and adjust the care plan as needed; * Serve as the primary liaison between family, multidisciplinary team, and health insurance provider; * Conduct home visits in specific cases, whenever necessary; * Ensure compliance with patient safety protocols, adverse event prevention, and nursing best practices; * Manage materials, supplies, and equipment required for home-based care; * Prepare progress reports and maintain accurate documentation in the medical record; * Interface with health insurance providers for authorizations and case billing. **Requirements and Qualifications** * Completed Bachelor’s degree in Nursing, with active registration at COREN; * Prior experience in home healthcare, ICU, internal medicine, or case management; * Knowledge of Nursing Care Systematization (SAE); * Strong communication skills and ability to interact sensitively with families during challenging times; * Organizational ability, capacity for prioritization, and sound decision-making under pressure; * Willingness to travel between patients’ residences.
