




Job Summary: Nurse focused on home care and patient navigation, responsible for ensuring quality of care, management of materials and medications, and humanized reception. Key Highlights: 1. Humanized reception and assurance of patient safety Description: Mandatory: * Bachelor's degree in Nursing; * Active registration with COREN; * Prior experience in home care (homecare) or patient navigation; * Knowledge of materials and medication management; * Ability to provide humanized reception and continuous patient monitoring. * Ensure management and monitoring of the patient portfolio under one's responsibility through efficient navigation and supply management, as well as beneficiary satisfaction and technical control; * Ensure quality of care provided to the patient and humanization of service; * Provide guidance to the beneficiary's caregivers whenever necessary; * Manage materials and medications in home care: control of material requests and releases, diet management; * Evaluate the quality of care provided to the beneficiary through navigation; * Execute and ensure excellent reception and navigation throughout the beneficiary's journey in the program, from admission to safe discharge; * Promote patient safety during the care process; * Conduct analysis of clinical indicators when requested by immediate management; * Perform other related duties at the discretion of the PGC's immediate management; * Participate in the development of standards, routines, technical procedures, and actions within one's competence in the PGC; * Maintain the physical infrastructure under one's responsibility; * Adhere to and commit to the institution's nursing and continuing education programs currently in effect; * Maintain contact with the multidisciplinary team in the area to enhance integration and service development; * Conduct and participate in meetings with team members according to the program protocol; * Participate in working groups focused on improving clinical practice and share experiences and best practices with the team, as well as participate in and audit navigations, home visits, materials and medication management, and discharge planning; * Keep the corporate email inbox and all available communication tools updated; * Ensure proper completion of controls in spreadsheets and reports essential for generating indicators and maintaining evidence of program management; * Enter material and medication requisition orders; * Meet navigation targets and other targets set by the nursing management. 2512160202201868420
