We seek students passionate about healthcare, interested in monitoring and assisting elderly individuals, contributing to rehabilitation, treatment, and disease prevention—always prioritizing quality of life and well\-being. **Your daily responsibilities will include:** * Conduct initial assessments for new patients, collaborating on personalized care plans under the supervision of the responsible nurse; * Conduct follow-up consultations, assisting in maintaining and updating care plans; * Respond to patient requests in person and by phone, providing guidance and support; * Administer and update clinical assessment tools, including the Clinical Frailty Scale, prediabetes assessments, Zarit Scale, cognitive tests, and the Geriatric Depression Scale (GDS); * Monitor and respond to messages via WhatsApp, ensuring clear and organized communication; * Schedule appointments and procedures, appropriately assigning them to responsible professionals; * Conduct follow-up calls with patients to monitor care progress; * Participate in meetings with the assigned nurse, contributing relevant information to continuously improve care delivery; **What we’re looking for:** * Undergraduate nursing students who have already completed the Technical Semiology course; * Responsible, committed, punctual individuals with a proactive attitude; **What we offer:** * Internship stipend aligned with market standards; * Transportation allowance. **Work schedule:** Monday through Friday, from 1:00 PM to 7:00 PM, plus one Saturday per month according to the roster.
