




Job Summary: Manage clinical and administrative care for patients at home, ensuring quality of care, effective communication, and excellence in service delivery. Key Highlights: 1. Clinical and administrative management of patients receiving home-based care. 2. Coordination with the multidisciplinary team and communication with family members. 3. Ensure clinical and administrative management of patients under follow-up care, guaranteeing quality of care, timely updates to care plans, alignment with the multidisciplinary team, and effective communication with family members and health insurance providers, aiming for continuity, safety, and excellence in patient care. **Responsibilities and Duties** * Manage active clinical cases and monitor patient progress. * Validate medical prescriptions, supplies, consumables, and care plans. * Promote continuous alignment with the multidisciplinary team. * Maintain contact with and provide support to family members and caregivers. * Monitor patients and identify clinical changes early. * Audit nursing reports and clinical documentation. * Follow critical cases and assisted weaning processes. * Prepare technical, managerial, and clinical assessment reports. * Submit requests for extensions and addendums to health insurance providers. * Enter information and technical documents into health insurance providers' portals. * Ensure organized shift handovers and safe continuity of care. **Requirements and Qualifications** * Bachelor's degree in Nursing with an active COREN registration. * Experience in Home Care, home-based hospitalization, or inpatient hospital care. * Knowledge of mechanical ventilation, tracheostomy, gastrostomy, and oxygen therapy. * Experience with care plans, medical record auditing, and health insurance providers. * Proficiency in electronic health records, hospital information systems, and Microsoft Office Suite. * Strong communication skills, organizational ability, proactivity, and analytical capacity. * Certifications in Home Care, ICU, nursing auditing, or healthcare management are advantageous. **Additional Information** Benefits: * Meal Allowance: BRL 255.95/month * Food Voucher: BRL 30.50/day * Transportation Allowance or on-site parking * SulAmérica Health and Dental Insurance Plan * Group Life Insurance * SESC Partnership * Daycare Assistance for mothers with children up to 6 years old * Wellhub Forget everything you know about healthcare.
