Come join our team and become a Med Member. **Responsibilities and Duties** Provide social work assistance—specifically to patients served at the Specialized Rehabilitation Center (CER/CEES)—in accordance with identified needs, aiming for comprehensive care for patients with physical, hearing, and intellectual disabilities; addressing motor, sensory, perceptual, and cognitive rehabilitation requirements; delivering assistance to all patients served, from least to most complex cases, based on technical and scientific knowledge, striving to ensure physical, mental, and social well-being; verify and record in patient records all information related to care delivery per service protocols, as well as any daily incidents concerning the patient; collaborate with the multidisciplinary team and jointly develop routines ensuring integration of Social Work services throughout the entire process—from admission (entry of user/family into the service) through discharge; identify and address social aspects presented by users to guarantee their participation in the rehabilitation process, as well as full awareness of their health status and discussion of their actual needs and recovery possibilities given their life circumstances; coordinate intra- and inter-sectorally with network points and services that respond to diverse and complex basic needs when constructing individualized therapeutic plans and tailored intervention proposals across all levels of care (primary and specialized); participate in systematic in-service training programs, attend courses and events, and conduct debates and workshops within the institution’s geographical coverage area; promote continuous updating, professional development, and ongoing education; supervise interns and students, and participate in clinical meetings and case discussions, maintaining information exchange with other professionals or services within the SUS Network and internal/external communities, regarding activities related to the specialty; carry out health education activities, guide and train users and the Care Network, focusing on functionality and autonomy, social security, and improved accessibility; perform all activities in compliance with Biosafety standards. Must also prepare documents, reports, official letters responding to external institutions and internal CEES/CER departments regarding services rendered, and draft normative instructions on procedures and services carried out in the Social Work department. **Requirements and Qualifications** Bachelor’s degree in Social Work; Specific Knowledge: 1. Social Assistance Policy (SUAS) and Health Policy (SUS); 2. Legislation and rights of persons with disabilities; 3. Ability to design and implement social projects; 4. Experience in social inclusion processes and coordination with service networks.
