Collaborative work within a multidisciplinary team **Nutritionist Position – Home Care** Cachoeiro de Itapemirim/ES We are seeking a Nutritionist to provide home-based care, focusing on continuous nutritional support for patients, ensuring clinical support, food safety, and improved quality of life. **Main Responsibilities:** * Conduct comprehensive nutritional assessments at the patient's home; * Develop individualized meal plans based on clinical condition; * Prevent and treat malnutrition, dysphagia, and other dietary disorders; * Recommend appropriate food consistency in collaboration with the healthcare team (e.g., speech-language pathology); * Monitor dietary acceptance and patient progress; * Educate family members and caregivers on diet preparation, administration, and management; * Manage oral diets, enteral nutrition (where applicable), and nutritional supplementation; * Document patient progress in medical records and generate reports; * Collaborate with a multidisciplinary team. **Requirements:** * Bachelor's degree in Nutrition * Active registration with the Regional Nutrition Council (CRN); * Clinical experience (home care experience is preferred); * Knowledge of clinical nutrition, especially for elderly and/or chronic patients; * Experience with modified diets and, if possible, enteral nutrition; * Strong communication skills, empathy, and a human-centered approach; * Availability for home visits.
