Together, let’s build the world’s best health community! **Responsibilities and Duties** * Provide periodic care to patients and their families; * Interview and guide patients regarding access to free transportation and related documentation, as well as obtaining high-cost medications; * Provide general assistance to families, identifying patients who miss appointments; * Refer patients with physical disabilities to appropriate agencies; * Facilitate dialysis slot allocation for patients in transit and/or transfer; * Collect and analyze required documentation for initiating patient treatment. **Requirements and Qualifications** * Completed undergraduate degree in Social Work; * Active and valid CRESS registration; * Availability to work 30 hours per week (150 hours per month); * Prior experience in clinical care; * Experience working with renal patients is desirable; * Knowledge of the INSS process is desirable. **Additional Information** **What We Offer?** * Health Insurance Plan: to take care of your well-being and that of your family.
