* Set up the implementation process, including health insurance documentation and implementation-related documentation;
* Carry out the implementation process, including: patient registration, budget estimation, and health insurance authorization codes;
* Request patient transfer upon contact with the family, hospital discharge, and approval of the therapeutic plan by the health insurance provider.
* Manage inpatient processes, such as scheduling examinations, transfers, and elective procedures.
* Record medical discharge;
* Transfer patients’ care programs;
* Schedule visits by the multidisciplinary team;
* Modify the Home Care Plan (PAD);
* Handle hospital discharges during weekend on-call shifts (discharge confirmation, patient transfer, and documentation verification);
* Generate reports for the multidisciplinary team;
Support patient monitoring via telephone.
