Job Summary:
Hospital Pharmacist responsible for the safe dispensing of medications, risk monitoring, supervision of storage conditions, and collaboration with the medical team to ensure patient safety.

**Responsibilities:**
* Accurately and safely dispense medications to patients according to medical prescriptions and established regulations.
* Monitor risks associated with the dispensing process (report, investigate, and propose improvements).
* Supervise appropriate medication storage, ensuring optimal temperature, humidity, and lighting conditions.
* Collaborate with medical and nursing teams to provide information regarding medications, dosages, and drug interactions.
* Perform clinical pharmacy activities with emphasis on continuous improvement and patient safety.
* Participate in clinical meetings to discuss cases and contribute pharmaceutical insights to treatment planning.
* Conduct medication reviews and reconciliations to identify and prevent medication errors.
* Provide guidance to patients and their families on proper medication use and potential side effects.
* Deliver training sessions to multidisciplinary staff.
* Perform other related duties as required by the operational unit’s responsibilities and the nature of the work, per management directives.
**Requirements:**
* Bachelor's degree in Pharmacy, with active registration at the Regional Council of Pharmacy (CRF);
* Specialization in Hospital Pharmacy;
* Experience in Clinical Pharmacy;
* **Working Hours: 7:00 AM to 7:00 PM. Shift schedule: 12 hours on / 36 hours off.**
**Preferred Qualifications:**
* Familiarity with the Tasy system;
* Basic computer skills and Microsoft Office Suite;
* Inventory management.
Employment Type: Full-time CLT
Compensation: R$4,000.00 – R$4,700.00 per month
