**Responsibilities and Duties** **Main Activities:**
* Conduct pharmacotherapeutic monitoring of inpatients, focusing on treatment efficacy and safety;
* Evaluate medical prescriptions regarding dosage, route of administration, frequency, potential drug interactions, and compliance with restricted-use protocols;
* Notify and investigate adverse drug reactions and medication-related adverse events;
* Participate in multidisciplinary rounds across clinical units;
* Advise healthcare staff on correct medication use, dilutions, incompatibilities, and infusion times;
* Promote rational drug use based on clinical protocols and guidelines;
* Contribute to the development, updating, and implementation of institutional pharmacotherapeutic protocols and guidelines;
* Provide medication counseling to patients and caregivers upon hospital discharge;
* Perform pharmaceutical anamnesis and collect complete pharmaceutical history within the first 48 hours after patient admission;
* Support pharmacovigilance and technovigilance activities in coordination with the Patient Safety Unit;
* Participate in hospital committees, such as the Pharmacy and Therapeutics Committee (PTC) and the Patient Safety Committee;
* Systematically document clinical activities in patient records or electronic health information systems;
* Assume technical and legal responsibility for all pharmacy-related incidents;
* Advise nursing and medical teams on proper use of prophylactic and therapeutic medications;
* Control and verify incoming and outgoing medications and materials stored in the department, ensure rational drug use, and conduct hospital pharmacy inventories;
* Monitor antibiotic prescriptions and their standardization;
* Monitor prescription workflows for pharmaceutical products and dispense controlled substances;
* Validate fractionation and labeling of oral solid dosage forms;
* Ensure supply of all medications required for patient care;
* Implement new processes introduced by pharmacy management, deploy work instructions, and monitor their effectiveness;
* Submit pharmacovigilance notifications;
* Perform other related tasks.
**Requirements and Qualifications** **Required Qualifications:**
* Bachelor’s degree in Pharmacy; postgraduate specialization in Clinical Pharmacy;
* Minimum 6 months’ experience in hospital or clinical pharmacy;
* Active registration with the Regional Council of Pharmacy (CRF);
* Intermediate proficiency in Microsoft Office Suite.
**Desirable:**
* Knowledge of clinical protocols, therapeutic guidelines, and patient safety;
* Experience in medium- or high-complexity hospital environments;
* Familiarity with electronic prescribing systems and electronic health records;
* Certifications or training courses in pharmacovigilance, rational drug use, or clinical pharmacy.
