Opportunity for professional development **Address** São Paulo, SP **Job Responsibilities** * Conduct pre-visit assessment and nursing visits to patients receiving home-based care, in accordance with the PAD therapeutic plan; * Perform clinical assessment of the patient, reviewing and validating the therapeutic plan and nursing prescription; * Supervise patient care to ensure quality and safety of assistance; * Guide and train caregivers and patients on self-care; * Organize, verify, and collect the patient’s medical record, ensuring complete and up-to-date documentation; * Record consultations, assessments, and progress notes on institutional forms and electronic systems within established deadlines; * Verify compliance related to materials, medications, equipment, medical records, and fall/injury prevention measures; * Check functionality, maintenance, concurrent cleaning, and terminal cleaning of equipment, furniture, and medical gases; * Manage bed-side material and medication inventory, performing returns using institutional forms; * Integrate into the multidisciplinary team, contributing to excellence in home-based care; * Participate in clinical meetings, medical record committees, mortality review committees, and other institutional meetings; * Notify the Patient Safety Unit in cases of incidents or adverse events; * Attend training sessions, capacity-building programs, and professional updating activities; * Deliver all required documentation evidencing patient care to the client within the established deadline. **Requirements** * Completed undergraduate degree in Nursing * Active and regular COREN registration * Minimum 2 years of experience in this role * Experience in Home Care * Valid driver’s license and personal vehicle **Additional Notes and/or Benefits** Transportation allowance, Meal/food allowance of R$25.00 per day, TotalPass, Birthday Day Off, Amil health insurance plan, Bradesco dental insurance plan **Organization’s Mission** "Transforming the post-acute healthcare journey by delivering humanized care and excellent clinical outcomes, adding superior value for our patients, employees, and all stakeholders in this chain." **About the Organization** Grupo Opalus is a dynamic and innovative holding company in the Brazilian healthcare sector, a leader in transitional care and post-acute assistance.
