




Job Summary: An administrative support professional for a multidisciplinary team, responsible for scheduling coordination and supporting provider engagement in transitional care. Responsibilities: * Prospect and support engagement with multidisciplinary team professionals through service providers, according to patient PADs; * Support the organization and retention of multidisciplinary team schedules; * Request, organize, and manage the schedule for multiprofessional appointments under coordination supervision; * Schedule and reschedule visits according to clinical needs and coordination instructions; * Monitor attendance, coverage, and timekeeping records of the multiprofessional team; * Provide administrative support to the multiprofessional team, guiding them on internal routines, document workflows, and form completion; * Assist the multidisciplinary team coordination in administrative, operational, and cross-departmental interface activities; * Guide patients, family members, and multidisciplinary team professionals as needed. Requirements: * Completed high school education; * Experience in home care and multidisciplinary scheduling; * Proficiency in Microsoft Office suite; * Preferred: familiarity with IW and experience in schedule management.
