




Description: Required: Bachelor's Degree in Pharmacy Preferred: Relevant experience * Technically supervise the department’s work, supported and assisted by the Co-Responsible Pharmacist and/or the Technical Responsible Pharmacist (TRP) of the unit; * Interface with other departments and submit activities to the immediate supervisor for approval, ensuring timely and efficient workflow; * Understand, interpret, and comply with legislation applicable to hospital activities and pharmaceutical care; * Participate in the control, use, and recording of medications listed in Ordinance No. 344/98 and other substances regulated by Sanitary Surveillance and other regulatory agencies; * Receive and technically evaluate medical prescriptions to verify their conformity, thereby facilitating prescription release; * Develop and discuss with the nutrition team the Drug–Nutrient Interaction Chart, ensuring no interactions occur that could adversely affect patients’ clinical progression; * Validate patients’ medications to ensure compliance with institutional criteria; * Actively participate in multidisciplinary patient rounds, discussing individual cases and proposing actions to support implementation of prescribed therapeutic plans; * Provide discharge counseling tailored to the unit’s epidemiological profile; * Prepare staff duty schedules, subject to validation by the TRP; * To hold the position of Pharmacist, the candidate must possess a valid registration with the Regional Council of Pharmacy (CRF) and must have fully settled all financial obligations with said council; * Perform other related duties as assigned by the supervisor. 2512110202491899091
