Opportunity to positively impact the health of the assigned population. * Join the Family Health Team (FHT), establishing and reinforcing the bond between individuals/families/community and health services; * Monitor families within their micro-area of operation, both at home and alongside other members of the Family Health Team (FHT), aiming to promote, prevent, and guide community health. **Responsibilities and Duties** * Participate in the process of territorialization and mapping of the team's area of operation, identifying groups, families, and individuals exposed to risks—including occupational risks—and continuously updating this information, prioritizing situations requiring follow-up in local planning; * Ensure comprehensive care through health promotion activities, disease prevention, curative actions, and guaranteeing access to spontaneous demand services; * Provide health care to the assigned population, primarily at the health unit, at home, and in other community settings (e.g., schools, associations) when necessary; * Conduct active case-finding and mandatory notification of diseases and conditions requiring compulsory reporting, as well as other locally significant health issues and situations; * Listen attentively to users’ needs in all activities, providing humanized care and facilitating the establishment of trust-based relationships; * Assume responsibility for the assigned population, even when they require care from another service within the health system; * Participate in team planning and evaluation activities using available data; * Promote community mobilization and participation, aiming to strengthen social control; * Identify community partners and resources that can enhance intersectoral actions with the team; * Ensure high-quality recording of activities in Primary Health Care information systems; * Participate in continuing education activities; * Develop actions aimed at integrating the health team and the population assigned to the UBS, considering the characteristics and objectives of monitoring individuals, social groups, or collectives; * Work with family listings based on a defined geographic area—the micro-area;; * Maintain continuous contact with families, delivering educational activities aligned with the team’s planning to promote health and prevent disease; * Register all individuals in their micro-area and keep registrations updated; * Guide families on how to use available health services; * Carry out health promotion, disease and condition prevention, and health surveillance activities via home visits and individual and collective educational actions in homes and communities, keeping the team informed—especially about those at risk; * Fulfill current responsibilities assigned to Community Health Agents (CHAs) regarding malaria and dengue prevention, as outlined in Ordinance No. 44/GM, dated January 3, 2002; * Monitor, via home visits, all families and individuals under their responsibility according to team-defined needs. **Requirements and Qualifications** * Completed high school education; * Prior experience in public-facing roles is desirable. **Additional Information** **UBSF MORADA NOVA** Monday to Friday, 07:00 to 17:00 Salary Range: R$2.600,00 to R$3.050,00 per month Benefits: Food Allowance: R$234.00 Life Insurance Partnerships with Tangará and Sesc Clubs Transportation Allowance **SPDM** – São Paulo Association for the Development of Medicine is a non-profit, philanthropic civil association recognized as being of federal, state, and municipal public utility, respectively by Decrees No. 57.925, dated March 4, 1966; No. 40.103, dated May 17, 1962; and No. 8.911, dated July 30, 1970. Their operations are dynamically aligned with the needs of Uberlândia Municipal Health Secretariat (SMS) to serve the population of Uberlândia/MG. SPDM – São Paulo Association for the Development of Medicine assumed management of Uberlândia’s Integrated Care Units (UAIs) on June 1, 2018, striving for excellence in public health services and commitment to SUS principles.
