




Description: * Bachelor's degree in Nursing, Social Work, or Administration; * Valid and active professional council registration; * Prior experience in nephrology; * Proficiency in Microsoft Office suite; * Patient-centered focus throughout the discharge process. * Responsible for contacting and welcoming the eligible patient and their family members for the discharge process; * Responsible for coordinating with the hospital and/or health insurance provider to ensure that patients eligible for discharge to a DaVita clinic have all exams required by RDC regulations; * Responsible for presenting DaVita clinics and the various treatment modalities to hospitalized patients and their families; * Responsible for sending documents and exam results to the clinic for eligibility review of the health insurance plan and clinical evaluation of the patient; responsible for scheduling admission with the patient, facilitating coordination among physician, hospital, clinic, and patient; * Responsible for requesting, through the commercial team, special authorization from the health insurance provider in cases where the patient is not yet credentialed; * Provide guidance to patients and families regarding necessary post-discharge care and the admission process; * Collaborate with the medical team to determine the optimal strategy for patient discharge; * Responsible for scheduling the first follow-up consultation when hospitalized patients require continued outpatient care; * Manage patient transfers to DaVita clinics pursuant to agreements or commercial demands; * Engage in patient acquisition from non-DaVita sources (external outpatient clinics and in-hospital services); * Maintain official records and enter data into the system; * Ensure all discharge requests and/or admission requests are fulfilled promptly, thereby ensuring not only external customer and patient satisfaction but also minimizing hospitalization duration; * Ensure compliance with all DaVita protocols and policies, including its Compliance policies. 2512200202551199375
