Your role is clinical, educational, logistical, and emotional—ensuring access, continuity, and safety. 1\. Patient Welcome and Education * Conduct initial welcome after oncological diagnosis, explaining the disease, treatment plan, and next steps. * Promote health education regarding treatment, side effects, self-care, and warning signs. * Ensure the patient understands their rights, available support channels, and how to access available services. 2\. Care Journey Coordination * Serve as the patient’s focal point throughout the entire treatment process: diagnosis, staging, active treatment, and follow-up. * Coordinate and monitor appointments: consultations, exams, infusions, hospitalizations, surgeries, and follow-up visits. * Conduct active follow-up via telephone, teleconsultation, or in-person visits, ensuring adherence and safety. 3\. Administration of Immunotherapeutics (In-House Network) * Supervise or administer immunotherapeutics within the in-house network, per clinical and pharmacological protocols. * Ensure compliance with safety criteria: correct patient identification, prescription verification, and pre- and post-infusion counseling. * Monitor and document immediate and delayed adverse reactions, notifying the medical and pharmacy teams. 4\. Dispensing and Monitoring of Oral Chemotherapy * Collaborate with the clinical pharmacy team to ensure safe dispensing of oral oncology medications. * Confirm eligibility and prior authorization per protocols, advising patients on dosage, storage, and toxicity warning signs. * Conduct active monitoring of therapeutic adherence, identifying and managing adverse effects and barriers to correct use at an early stage. 5\. Monitoring of Events and Outcomes * Participate in longitudinal monitoring of clinical outcomes, such as treatment response, avoidable hospitalizations, and adverse events. * Support the audit and technical authorization team with clinical insight and knowledge of the patient’s history. * Enter data into systems (BI, Tasy, electronic medical record) regarding progress notes, support provided, and interventions performed. 6\. Psychosocial Support and Humanization * Identify signs of emotional distress, social barriers, and uncertainty about treatment. * Coordinate with the multidisciplinary team (psychology, nutrition, social work) and promote person-centered care. * Maintain continuous engagement with the patient and their family, reducing treatment abandonment and unplanned interruptions.
