Standardize, develop, and document area-specific procedures, ensuring compliance with institutional guidelines and current legislation; 2. Conduct individual and/or group interventions aimed at addressing the social determinants of health among users (patients, families, and companions); 3. Guide individuals and groups from diverse social segments to identify available resources and effectively utilize them in service delivery and rights advocacy; 4. Provide periodic social assistance to patients, including reception, qualified active listening, and social assessment; 5. Develop and update the area-specific procedural manual, ensuring implementation of best practices in social services; 6. Participate in daily clinical meetings with the multidisciplinary team, providing information on patients’ social needs and contributing to the formulation of appropriate care strategies; 7.
